Początki Atlético B sięgają roku 1970, kiedy to oficjalnie stało się ono filią Rojiblancos. Było to efektem połączenia Reyfra Atlético Club (utworzonego w 1966 roku satelickiego zespołu Colchoneros) i Aviaco Madrileño CF. Nowy twór został nazywany Atlético Madrileño Club de Fútbol i rozpoczął zmagania od Tercera División.
Na początku lat 90. zmiany w hiszpańskim prawie sportowym spowodowały, że kluby musiały zostać przekształcone w sportowe spółki akcyjne oraz posiadać drużyny rezerw. Ostatecznie Atlético Madrileño Club de Fútbol przekształciło się wówczas w Atlético B i od sezonu 1992-93 grało już nie jako niezależny klub, ale jako część Rojiblancos.
Największym sukcesem jest spędzenie jedenastu sezonów w Segunda División. W sezonie 1998-99 udało się nawet zająć w niej drugie miejsce, ale ponieważ pierwszy i drugi zespół nie mogą występować na tym samym poziomie rozgrywkowym, awans do Primera División był niemożliwy. Do ciekawej sytuacji doszło także na przełomie wieków. Kampania 1999-2000 zakończyła się siedemnastym miejscem na zapleczu hiszpańskiej ekstraklasy, ale ponieważ Rojiblancos spadli wówczas do drugiej ligi, rezerwy musiały zostać oddelegowane poziom niżej.
W Segunda División B z różnym skutkiem rezerwy spędziły piętnaście kolejnych sezonów, po których spadły w końcu do Tercera División. Wrócić do trzeciej ligi udało się po dwóch latach, dzięki zwycięstwu w swojej grupie i udanym przejściu przez baraże.
Kadrę Atlético B tworzą głownie najbardziej utalentowani piłkarze ze szkółki w wieku 19-21 lat oraz piłkarze sprowadzani z zewnątrz, którzy mają 19-25 lat.
